Dear colleagues and friends,
It is a genuine pleasure to welcome you to Bratislava and to bring together such an exceptional group of clinicians, researchers, partners and collaborators. Located on the Danube in the heart of Central Europe, Bratislava provides a fitting setting for open discussion and new ideas.
The treatment of acute heart attacks is one of the great successes of modern cardiovascular medicine. Reperfusion therapies, regional STEMI networks and dedicated clinical teams save lives every day. These achievements reflect decades of work by many of the clinicians and researchers represented at this meeting.
We have life-saving treatments—but they can only help when the right patient is recognized and referred in time. The first assessment may take place in an ambulance, emergency department or hospital without immediate access to expert ECG interpretation. Decisions must be made quickly and often with limited information. Missed occlusions can delay treatment, while false-positive activations may expose patients to unnecessary procedures and place additional pressure on clinical teams.
The STEMI paradigm transformed care by connecting a recognizable ECG pattern with a clear and urgent treatment pathway. It has saved countless lives and remains fundamental to contemporary practice. However, acute coronary occlusion is a dynamic disease. Occlusion, spontaneous reperfusion, re-occlusion and evolving ischemia can produce varied clinical and ECG presentations that cannot always be captured by a single pattern or millimetre threshold.
The emerging OMI paradigm offers an opportunity to build on this legacy. It encourages us to reconsider first-contact assessment, referral pathways, cath-lab activation, pharmacological treatment, the timing of angiography and the design of future clinical trials. This wider perspective may open new areas of research and help us ask more meaningful clinical questions.

Artificial intelligence can help us move towards this goal by making advanced ECG interpretation more consistently available. Its purpose is not to replace clinical judgement, but to help clinicians navigate complexity, recognize patients who may otherwise be missed and make faster, better-informed decisions.
Over these two days, I hope we can explore these opportunities openly, learn from one another and identify the questions that deserve our collective attention.
