Senior Backend Engineer

📍 Bratislava, Slovakia | 🕐 Full-time

About Powerful Medical

We build life-saving tech powered by AI. Our certified product PMcardio helps to diagnose cardiovascular diseases—the #1 cause of death globally—by analyzing ECGs. We’re backed by leading European, US, and Israeli venture capital firms and work closely with top healthcare providers across the EU and the United States.

About the Role

We’re hiring a Senior Backend Engineer to build the reliable, secure, and scalable systems behind our products. You’ll design and develop the API backend that powers PMcardio – our product used by thousands of doctors every day, ensuring it meets the high standards required for medical-grade software.

You’ll work closely with Product, Frontend, and AI to ship new features, improve system reliability, and shape backend architecture that scales with our growth. This is a role for someone who wants real ownership: making architectural decisions, solving hard backend challenges, and building production systems where reliability, security, and correctness truly matter. We are changing cardiology healthcare globally. If you want to solve meaningful engineering challenges and help bring AI into everyday clinical practice, this role is for you.

Your role 🚀

  • Design and build reliable, secure, and scalable backend services and APIs
  • Own architectural decisions for core backend systems and help shape how our platform evolves
  • Design data models and database schemas that remain robust as the product and team scale
  • Collaborate closely with Product, Frontend, and AI teams to deliver new features end to end
  • Build systems that handle scale, concurrency, asynchronous workloads, and high availability in production
  • Drive improvements in reliability, performance, and maintainability across the backend stack
  • Write automated tests and help raise the bar for backend best practices and engineering standards

Your profile 💡

  • Proven experience designing and scaling backend systems used by large numbers of users
  • Strong understanding of scalable system design, backend architecture, and database-backed applications
  • Experience owning technical decisions and building systems that need to be reliable, secure, and maintainable over time
  • Comfortable dealing with concurrency, asynchronous processing, and distributed system challenges
  • Strong Python experience in production; experience with typed Python and frameworks such as FastAPI is a plus
  • Strong engineering judgment, clear communication, and a collaborative mindset when working across teams

Monthly salary: from 4,500 EUR (depending on seniority) + stock options

Heart Attacks are #1 cause of death world-wide and killing about 12 milions people a year.

Clinical Definition of Problem

Contrary to popular belief, a heart attacks isn’t a blockage inside of the heart. A heart attack is a blockage of the coronary arteries supplying the heart muscle with oxygenated blood.

So let’s assume you get a blood clot here — it blocks the blood flow downstream, meaning the heart muscle doesn’t get oxygenated blood and heart tissue downstream starts to die.

Clinical Solution

The way to fix it is relatively simple – doctors put in a stent that opens up the artery and renews blood flow. The latest clinical practice guidelines recommend that this “stenting” happens within 90 minutes from symptom onset.

If you don’t, even if you put in the stent in later, the heart tissue downstream has already been permanently damaged, which reduces the heart’s ability to pump blood. This is the leading cause of heart failure and increases 1-year mortality by two-fold.

Time is muscle.

You have just 90 minutes to diagnose the patient, bring them to the hospital and put in the stent, otherwise there is permanent damage. So problem is, that 1 in 2 heart attacks get initially misdiagnosed at the first point of contact.
